N’oubliez pas d’imprimer la totalité des documents, de les remplir et de les signer avant de venir et d’ajouter l’annexe (carte de vaccination de l’enfant). Veuillez également imprimer, lire et parapher le contrat de collaboration et remettre une copie le jour de votre rendez-vous.

Do not forget to print all of the documents, to fill them out and to sign them before coming and to add the annex (vaccination card of the child). Please also print, read and initial the collaboration contract and hand in a copy on the day of your appointment.
