Are your minor children going on an excursion out of the country or on holiday with a third party?
Then they need a document called a parental authorisation. In principle, this document is a legalisation of the signature of a parent or legal guardian who is in a position to authorise this trip for the minor child.
In order for this document to be issued, the authorising parent/guardian must appear in person at the population office with his or her identity document.
The form below can be downloaded and completed in advance, without a signature! Please then go to the population office in person (for more information, please see the "legalisation of signature" section below).
